Social worker

• Conduct thorough mental health and risk assessments
• Develop and implement individualized care plans
• Work closely with a range of professionals
• Identify and respond to safeguarding concerns
• Facilitate access to community resources
• Provide timely and effective interventions during periods of crisis
• Maintain accurate and up-to-date records

• Degree in Social Work or equivalent
• Current registration with Social Work England
• Minimum of 2 years post-qualification experience in adult social work, with a focus on mental health and older adults
• In-depth understanding of relevant legislation, including the Mental Health Act, Mental Capacity Act, and Care Act
• Excellent verbal and written communication abilities
• Knowledge of safeguarding procedures
