Job Description:

As a Social Worker in the Adult Safeguarding Team, you will be essential in protecting vulnerable adults and promoting their safety and well-being. Your role involves conducting thorough risk assessments, implementing targeted safeguarding measures, and protecting adults at risk of abuse or neglect. You will work closely with individuals, their families, and multidisciplinary teams to provide effective, person-centered support that aligns with safeguarding protocols.

Job Responsibility:

Conduct comprehensive assessments of adults' needs in accordance with the Care Act 2014 and other applicable legislation
Create and implement personalised care plans designed to promote independence and enhance community living
Safeguard vulnerable adults by ensuring their safety and well-being through appropriate interventions
Collaborate with multidisciplinary teams, families, and community services to deliver holistic support
Advocate for the rights of adults, ensuring they have access to essential resources, services, and benefits
Regularly review and monitor care plans to accommodate changing needs and circumstances
Ensure adherence to statutory responsibilities and best practices in safeguarding and care delivery

Requirements:

2 years experience post qualification
Action safeguarding procedures
