The social worker provides clinical social work services to adult patients and their families in ambulatory clinics. Social work services provided include psychosocial assessments, information and referral, psychosocial treatment and other services required to optimize the patients' health care. The social worker collaborates with the health care team in the development and implementation of the care plan of the patients with whom the social worker is active. The social worker also provides consultation to the health care providers regarding patients' care needs and/or community resources.
Job Responsibility:
Performs psychosocial assessments of patients and families
Provides social work service to patients with problems identified through psychosocial assessments
Collaborates with other clinic care providers in development and implementation of interdisciplinary care plans
Coordinates referrals to and acts as liaison to community agencies regarding services to individual patients
Provides internal and external systems advocacy as indicated in care plans of individual patients
Maintains current knowledge of community resources relevant to the clinic patient population
Provides appropriate assessment and intervention services for children and adults suspected of being victims of abuse, neglect and/or domestic violence
Handles risk management issues
Documents social work activity in the electronic medical record
Provides social work coverage, as requested, to other clinical units or programs during co-workers' absence
Completes required in-service training programs and ongoing professional development activities
Completes required Employee Health appointments
Attends all appropriate Social Work Division meetings
Supervises social work students (MSW or BSW)
Orients and provides ongoing peer support to new social work staff
Requirements:
MSW from an accredited social work graduate program
Must obtain New York State license within eighteen months of employment
Prior clinical experience in a medical setting and supervisory experience preferred
Familiar with medical social work and community resources preferred
LMSW - Licensed Medical Social Worker - State Licensure within 1-1/2 years required
If duties will be performed under OMH a minimum of a limited permit (not to exceed 1 year) or NYS Licensure upon hire required
Nice to have:
Prior clinical experience in a medical setting and supervisory experience
Familiar with medical social work and community resources
