Job Description:

We are seeking a compassionate and proactive Social Worker to join a Hospital Discharge team. This is a vital role, ensuring seamless and safe transitions for patients leaving hospital, and providing crucial support to both individuals and their families. This opportunity allows you to work within a dynamic healthcare setting in the heart of Falkirk while contributing to a collaborative team

Job Responsibility:

Conduct comprehensive social work assessments to identify patients' needs and develop personalised discharge plans
Collaborate effectively with multidisciplinary teams, including medical staff, nurses, therapists, and community services
Arrange appropriate care packages, including home care, equipment, and financial support
Provide information, advice, and advocacy to patients and their families
Safeguard vulnerable adults and children, adhering to relevant legislation and policies
Maintain accurate records and contribute to service development
Work to prevent delayed discharges and promote patient independence

Requirements:

Qualified Social Worker
SSSC registered as a Social Worker
Car driver
At least 3 years hospital discharge experience
