Social Worker

• Conducting comprehensive assessments of needs, strengths, and risks for individuals with learning disabilities and/or autism
• developing and reviewing personalised care and support plans that promote independence, well-being, and community inclusion
• working collaboratively with individuals, their families, carers, and a range of multi-agency partners
• safeguarding vulnerable adults and responding effectively to safeguarding concerns
• navigating complex transitions
• providing expert advice and guidance on relevant legislation
• advocating for the rights and choices of individuals with learning disabilities and autism

• Qualified Social Worker
• substantial experience working with adults with learning disabilities and/or autism
• deep understanding of challenges and complexities faced by this client group
• expert knowledge of relevant legislation, policy, and best practice in adult social care, particularly concerning learning disabilities and autism
• excellent assessment, analytical, and communication skills
• proactive, empathetic, and resilience approach
• current registration with Social Work England
