Job Responsibility:
Assessment & Care Planning: Conduct comprehensive needs assessments (including Single Shared Assessment) and risk assessments for young adults and adults with physical and learning disabilities. Develop and review person-centred support and care plans
Transition Support: Lead and coordinate the transition process for young people moving from children's to adult services, ensuring a smooth and successful experience
Safeguarding: Carry out safeguarding duties, including adult protection investigations, in line with local procedures and legislation
Case Management: Manage a complex caseload, utilising effective planning, intervention, and review strategies
Resource Allocation: Identify and arrange appropriate support services, personal budgets, and community resources
Multi-Agency Working: Collaborate effectively with families, carers, health professionals, housing, education, and third-sector organisations
Legal Compliance: Ensure all practice is compliant with relevant legislation, including the Social Work (Scotland) Act 1968 and the Adults with Incapacity (Scotland) Act 2000
Requirements:
A degree in Social Work or an equivalent qualification recognised by the Scottish Social Services Council (SSSC)
Current SSSC registration as a social worker
