Job Description:

Casual roles for Flexible, experienced healthcare Social Workers in Palliative Care, Rehab and Older Persons Mental Health areas. The role involves providing high quality, comprehensive social work services during periods of planned or unplanned staff leave.

Job Responsibility:

Provide a holistic psychosocial perspective of the individual
Provide skilled, effective and timely social work interventions
Participate as an effective member of the multi-disciplinary team
Ensure all documentation, communication and policy requirements are met
Maintain strong links with relevant staff from health and community services
Actively engage with the Social Work Manager and team
Participate as a member of the HammondCare team in the application of the organisation’s mission and values

Requirements:

Recognised tertiary Social Work qualification and eligibility for AASW membership
Current working rights in Australia
Commitment to engage and align with HammondCare’s Mission in Action
Experience in providing Social Work Services in a hospital or community health setting
Knowledge and experience in providing a range of social work interventions
Knowledge of relevant community services and referral systems
Demonstrated effective, concise communication and strong interpersonal skills
Demonstrated self-motivation, organisation and time management skills
Flexibility and adaptability within different working environments and teams
Current unencumbered Drivers License and safe driving record
Can commit to blocks of casual work with a minimum 3 days weekly

Nice to have:

3 years post graduate social work experience
Experience in using NSW Health EMR - electronic medical record program
Fluency in languages other than English
