Job Responsibility
Manage a community caseload of adults with diverse needs, including physical disabilities, older age, and complex long-term health conditions
Conduct robust, holistic Care Act (2014) assessments and design creative support plans centered around independence
Lead on statutory Safeguarding Adults inquiries, ensuring vulnerable citizens are protected while promoting their autonomy
Collaborate closely with local health partners, occupational therapists, and housing teams to provide seamless integrated care
Requirements
A qualified, Social Work England-registered Social Worker
Proven experience working within a statutory Adult Social Care framework (Community, Integrated, or Locality teams preferred)
A solid understanding of the Care Act 2014, Mental Capacity Act 2005, and safeguarding protocols
A passionate advocate for independence who can communicate warmly with service users and build fast rapport
