Job Responsibility:
Provide person-centered support: Assess the needs of individuals and their families, creating care plans tailored to their circumstances

Safeguard vulnerable adults: Respond to safeguarding concerns with professionalism and care, ensuring that those at risk are protected
Work collaboratively: Build strong relationships with partners across health, housing, and community services to deliver seamless support
Promote resilience: Empower clients by fostering independence and supporting them to access community resources
Requirements:
Qualified Social Worker
SSSC registered in the Social Work part of the register
