An exciting opportunity has arisen for a compassionate and motivated Social Worker to join our friendly and highly specialist Cystic Fibrosis (CF) and Metabolic Bone teams, who is passionate about working with children, young people and their families in delivering high-quality, family-centred care within a multidisciplinary setting.
Job Responsibility:
Provide specialist social work support to children, young people and families affected by long-term health conditions
Provide emotional, practical and professional guidance, promoting resilience and wellbeing
Undertake holistic assessments and develop family-centred care plans
Work collaboratively within a multidisciplinary team to deliver coordinated, high-quality care
Advocate for children and families to ensure their voices inform decision-making
Support families through key stages including diagnosis, hospital admissions and transition to adult services
Maintain accurate records and contribute to service development and quality improvement
Requirements:
Qualified Social Worker with current Social Work England registration
Experienced in working with children and families, ideally within health or specialist services
Confident in managing complex cases and working with long-term or life-limiting conditions
A strong communicator who values teamwork and partnership working
Compassionate, resilient and committed to delivering high standards of care
Legal Frameworks relevant to Children's Social Care and to CAMHS / Mental Health
