Randstad are recruiting for Qualified Social Workers to join East Lothian Council's Adults Service in their Home First team. This is a full time role offering a fantastic competitive pay rate. This role is designed for a fast-thinking, agile Social Worker who thrives in a multidisciplinary hospital environment.
Job Responsibility:
Stabilising crisis situations, preventing hospital admissions, and ensuring that "home" is always the first consideration for patients in acute and community hospital settings
Work directly within Emergency Departments (RIE and ELCH) to provide immediate social work interventions, preventing unnecessary ward admissions through rapid crisis support and community resource mobilisation
Attend and contribute to daily Multi-Disciplinary Team (MDT) screenings to respond to urgent referrals, providing professional social work insight to stabilize high-risk situations
Ensure the "Home First" principle is upheld, advocating for individuals to be assessed in their own environment rather than a hospital ward to promote better long-term outcomes
Deliver focused, intensive support to bridge the gap between hospital discharge and long-term community care
Facilitate safe, timely transitions from hospital to home, ensuring that long-term care needs are assessed in the most appropriate setting, the patient's own home
Use professional judgment to make high-stakes decisions in a fast-moving, unscheduled care environment where throughput is essential to hospital flow
Work hand-in-hand with GPs, District Nurses, Occupational Therapists, and Hospital Consultants to create seamless pathways between acute health and social care
Assist in reducing "Length of Stay" (LoS) and preventing delayed discharges by identifying social barriers to health clearance early in the patient journey
Maintain a firm grasp of the Adult Support and Protection (Scotland) Act and Adults with Incapacity legislation within a crisis context
Record high-quality, concise data on MOSAIC to ensure the MDT has real-time access to social work progress and discharge plans
