The social worker is responsible for providing clinical services to patients admitted to their assigned units and their families. The social worker serves as a member of the interdisciplinary health care team on their assigned unit and serves as a liaison to health and social service agencies in the community.
Job Responsibility:
Provide clinical services to patients admitted to assigned units and their families
Serve as a member of the interdisciplinary health care team
Serve as a liaison to health and social service agencies in the community
Coordinate discharge planning for patients determined to be at psychosocial risk
Assist patients in navigating the health care system
Provide psychosocial support for patients
Serve as a resource consultant to patients and their families
Facilitate referrals to appropriate community agencies and hospital resources
May make home visits to provide essential psychosocial assessments and social work interventions
Provide social work interventions including risk screening, psychosocial assessments, crisis intervention, supportive counseling, and grief and bereavement counseling
Handle risk management issues in consultation with supervisor and Associate Director
Document social work activity in the electronic medical record
Provide social work coverage to other clinical units or programs during co-workers' absence
Complete required Hospital and SW Division in-service training programs and ongoing professional development activities
Complete required Employee Health appointments
Attend appropriate Social Work Division meetings
Supervise social work students (MSW or BSW)
Orient and provide ongoing peer support to new social work staff
Requirements:
MSW from an accredited social work graduate program
Must obtain New York State license within eighteen months of employment
Prior clinical experience in a medical setting and supervisory experience preferred
Familiarity with medical social work and community resources preferred
LMSW - Licensed Medical Social Worker - State Licensure within 1-1/2 years required or a minimum of a limited permit (not to exceed 1 year) or NYS Licensure upon hire if duties will be performed under OMH
Nice to have:
Prior clinical experience in a medical setting
Supervisory experience
Familiarity with medical social work and community resources
