Job Responsibility:

  • Provide essential support for adult service users, ensuring safe and timely discharges from hospital
  • Work closely with Peterborough City Hospitals multi-disciplinary teams to enable discharges within the Discharge to Assess framework

Requirements:

  • Current registration with Social Work England
  • Completed AYSE
  • In-depth knowledge of current legislation and guidance, as well as current national policy and practice developments
  • Full Drivers Licence and the ability to travel between different locations
  • Willing to learn new things and able to maintain your own continuing professional development
