Job Description

We are looking for a dedicated and dynamic Locality Social Worker to join our team. The ideal candidate will have a passion for supporting individuals and families within their community, providing comprehensive case management, and working collaboratively with other professionals. This role requires excellent communication skills, a strong understanding of social work principles, and the ability to manage a diverse caseload effectively. Up to £35 per hour dependant on expereince.

Job Responsibility

  • Assess the needs of individuals and families within the community
  • Develop and implement personalized care plans
  • Monitor and review progress, making adjustments to care plans as necessary
  • Maintain accurate and up-to-date case records
  • Provide emotional and practical support to service users
  • Conduct home visits, assessments, and reviews
  • Facilitate access to resources and services, such as housing, healthcare, and financial assistance
  • Advocate for the rights and needs of service users
  • Work collaboratively with other social workers, healthcare professionals, educators, and community organizations
  • Participate in multidisciplinary meetings to ensure coordinated care
  • Develop and maintain strong relationships with local agencies and service providers

Requirements

  • Minimum of 2-3 years of experience in social work, preferably within a community or locality setting
  • Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited institution
  • Licensed Social Worker (LSW) or equivalent certification as required by state law
