We are looking for a dedicated and dynamic Locality Social Worker to join our team. The ideal candidate will have a passion for supporting individuals and families within their community, providing comprehensive case management, and working collaboratively with other professionals. This role requires excellent communication skills, a strong understanding of social work principles, and the ability to manage a diverse caseload effectively.
Job Responsibility:
Assess the needs of individuals and families within the community
Develop and implement personalized care plans
Monitor and review progress, making adjustments to care plans as necessary
Maintain accurate and up-to-date case records
Provide emotional and practical support to service users
Conduct home visits, assessments, and reviews
Facilitate access to resources and services, such as housing, healthcare, and financial assistance
Advocate for the rights and needs of service users
Work collaboratively with other social workers, healthcare professionals, educators, and community organizations
Participate in multidisciplinary meetings to ensure coordinated care
Develop and maintain strong relationships with local agencies and service providers
Requirements:
Minimum of 2-3 years of experience in social work, preferably within a community or locality setting
Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited institution
Licensed Social Worker (LSW) or equivalent certification as required by state law
