Job Description

Service Care Solutions are currently recruiting for a Social Worker to join the Adults Learning Disabilities Community Care Team within the London Borough of Merton.

Job Responsibility

  • Undertake the full Care Management process in line with the Care Act and statutory guidance
  • Manage a caseload as allocated by the Team Manager, maintaining accurate and up-to-date records (paper and IT systems)
  • Complete risk assessments in line with Merton Care Management Procedures
  • Deliver duties under relevant legislation including the Mental Capacity Act and Deprivation of Liberty Safeguards (DoLS)
  • Support safeguarding enquiries and promote Making Safeguarding Personal
  • Commission care packages, ensuring value for money and quality service delivery
  • Maintain accurate records on CareFirst and other systems
  • Attend meetings, court hearings, and tribunals as required
  • Engage in joint working with partner agencies and stakeholders
  • Contribute to service development, data reporting, and continuous improvement

Requirements

  • Qualified Social Worker with relevant registration
  • Experience working within Adult Social Care, ideally Learning Disabilities
  • Knowledge of the Care Act, MCA, DoLS, and safeguarding processes
  • Strong assessment, communication, and case management skills
  • Ability to work both independently and as part of a team
