As our Social Worker you will proactively assess, develop, and implement comprehensive plans of care in collaboration with patients, families, and an interdisciplinary team.
Job Responsibility
Complete psychosocial assessments, interview patients, relatives, friends or significant others to obtain information about the patient’s personal, social and emotional history
Provide therapeutic care to patients to assist them in accomplishing treatment goals using interventions and techniques that are appropriate to the age and developmental level of the patient
Coordinate treatment with community agencies such as social services, schools and probation
Maintain social service records of each patient according to hospital and departmental program standards
Maintain statistics and turn in reports in a timely manner
Communicate with crisis team for mental health emergencies
Coordinate outpatient treatment following inpatient hospitalization
Assist in revision of policies and provide input to the departmental budget by identifying departmental supply and equipment needs to meet departmental goals

Requirements
Bachelor's Degree in Social Work
Licensed Clinical Social Worker: MN (LCSW:MN)
Master's Degree in Social Work
