As a Social Worker, you’ll serve as a trusted counselor, crisis responder, advocate, and systems‑thinker—conducting psychosocial assessments, coordinating care, removing barriers, and ensuring every patient receives timely support and a safe, effective plan for life beyond the hospital. Job Responsibility:
Complete comprehensive psychosocial assessments focused on post‑hospital needs
Provide counseling and crisis intervention
Partner with Clinical Care Management to evaluate and place patients
Serve as a clinical resource for clinicians and physicians
Educate patients and families on community resources and post‑hospital options
Report suspected child or adult abuse, neglect, or exploitation
Respond to consult requests after normal business hours as part of scheduled on‑call coverage
Advance patient throughput and discharge planning
Promote organization‑wide quality improvement
Serve as a key information and education resource for the interdisciplinary team
Contribute to optimal clinical and resource outcomes
Requirements:
Master’s Degree in Social Work (MSW)
LCSW preferred
applicable state certification preferred
Minimum of 1 year of professional experience in a hospital or health‑related setting preferred
Experience with data management tools
proficiency in electronic medical documentation preferred
Adherence to the National Association of Social Workers (NASW) Code of Ethics
