Job Responsibility
Carry out robust statutory assessments within a fast-moving hospital environment
Design, implement, and monitor creative care packages that support the 'Home First' ethos
Navigate complex legal and safeguarding frameworks, including Adult Support and Protection (ASP) and Adults with Incapacity
Facilitate multidisciplinary discharge planning meetings to ensure all aspects of an individual's care are coordinated
Requirements
Must be a qualified Social Worker registered with the SSSC (Scottish Social Services Council)
Background working within adult social care, a hospital setting, or a fast-paced community assessment team
Confident knowledge of sectioning, care procurement, and risk management in acute or transitional environments
