Job Responsibility

  • Carry out robust statutory assessments within a fast-moving hospital environment
  • Design, implement, and monitor creative care packages that support the 'Home First' ethos
  • Navigate complex legal and safeguarding frameworks, including Adult Support and Protection (ASP) and Adults with Incapacity
  • Facilitate multidisciplinary discharge planning meetings to ensure all aspects of an individual's care are coordinated

Requirements

  • Must be a qualified Social Worker registered with the SSSC (Scottish Social Services Council)
  • Background working within adult social care, a hospital setting, or a fast-paced community assessment team
  • Confident knowledge of sectioning, care procurement, and risk management in acute or transitional environments
  • Excellent negotiation and interpersonal skills
