The Hospice Social Worker supports patients, caregivers, and families by addressing the emotional and psychosocial needs that arise with advanced illness and end-of-life care. This role works closely with the interdisciplinary team to develop and support an individualized plan of care. The Social Worker provides counseling, assessments, interventions, and resource coordination to help families navigate challenges and maintain safety and well-being in the home setting.

Requirements:
Bachelor of Science in Social Work, Psychology, or Sociology
MSW preferred
At least one year of experience in a health-care setting
hospice experience preferred
Understanding of hospice philosophy and issues related to death and dying
Strong communication, documentation, and organizational skills
Ability to travel throughout the Blue Ridge Care service area in Loudoun County
