Job Description
Fixed-term Social Worker secondment for six months within the Community Mental Health Team for Older Adults. The team champions a strength-based approach, placing individual independence at the centre of practice and supporting vulnerable residents.
Job Responsibility
Provide advice, information, support and care to people with diagnosed memory problems and those with mental health needs requiring social care
Help coordinate available resources to identify individuals requiring longer-term support and deliver short-term interventions using strengths-based assessments
Support individuals with social care needs to assess their own requirements and develop person-centred support plans
Take responsibility for assessing safeguarding referrals including carrying out investigations, completing risk assessments, chairing meetings
Establish good clinical and management practices ensuring high standards of care
Requirements
Compassionate and motivated professional with strong commitment to person-centred care
Skilled at building relationships across health, social care and voluntary sector
Organised, adaptable and comfortable managing complex situations including safeguarding responsibilities
Experience of working within the current Social Care System and Framework
Must reside in the UK
