Job Responsibility:
Conducting statutory assessments (e.g., Section 17, Carers Assessments) to determine eligibility for services
Developing and implementing comprehensive, personalised care packages, including short breaks, direct payments, and resource allocation
Leading transition planning for young people moving from children's to adult services
Working closely with the SEND (Special Educational Needs and Disability) framework and EHC (Education, Health and Care) Plans
Managing complex safeguarding concerns, including Deprivation of Liberty (DoLs) matters for 16/17-year-olds
Handling Child Protection (CP) and Child in Need (CIN) cases within the CiWD cohort
Liaising effectively with Health, Education, and specialist voluntary sector partners
Requirements:
A recognized professional qualification in Social Work
Current registration with Social Work England (SWE)
A minimum of 2 years post-qualified experience, ideally within a CiWD or specialist disability team
Expert knowledge of the Children Act 1989 and relevant parts of the Care Act 2014 (transitions)
Ability to manage a complex caseload autonomously
Full UK Driving Licence and access to a vehicle are essential for traveling across the Carlisle locality
