Job Description:

As a Social Worker in thier Locality Team, you will provide high-quality, strengths-based assessments that maximise an adult's involvement and choice.

Job Responsibility:

  • Assessment & Eligibility: Identifying levels of need under the Care Act 2014 across various settings (community, hospital, and residential)
  • Specialist Practice: Undertaking Mental Capacity, Best Interest, and Risk Assessments in line with the Mental Capacity Act 2005
  • Caseload Management: Managing an active caseload that matches your skill level and professional development goals
  • Advocacy & Support: Supporting the use of Direct Payments and Self-Directed Support to give residents true control over their care
  • Collaboration: Working alongside partners in health and housing, chairing safeguarding meetings, and preparing reports for the Court of Protection

Requirements:

  • Qualified: A Social Work qualification (MA/BA, Dip SW, CQSW, or CSS) or currently in your final year of study
  • Registered: Current registration with Social Work England (or eligibility to register/return to practice)
  • Legislative Expertise: A solid understanding of the Care Act, Mental Capacity Act, Mental Health Act, and Safeguarding principles
  • Strengths-Based Approach: Ability to apply social work theory to person-centred practice
  • Proven experience working with adults (and carers) living with physical disabilities, mental health needs, or learning disabilities
  • A track record of managing risk and participating in the safeguarding process
  • Experience in evidence-based decision-making and workload prioritisation
  • Communication: Ability to build rapport with a diverse range of people and professionals
  • Digital Literacy: Strong IT skills for maintaining accurate records in line with GDPR
  • Mobility: A current driving licence and access to a car (or equivalent mobility support)
