Job Responsibility:
Being the first point of contact for new referrals and urgent situations, providing timely and effective responses
Conducting initial assessments to understand immediate needs and risks
Providing advice, information, and support to individuals, families, and partner agencies
Developing and implementing short-term action plans to address immediate concerns
Safeguarding vulnerable adults and children in line with local procedures and legislation
Working collaboratively with other professionals, including health, education, and police colleagues
Maintaining accurate and up-to-date records
Participating in team meetings and contributing to service development
Requirements:
A recognised Social Work qualification (e.g., BA/MA in Social Work, DipSW)
Registration with the Scottish Social Services Council (SSSC) as a Social Worker
A full driving licence and access to a vehicle
Excellent communication skills as the work involves multi-agency liaison and the ability to establish effective working relationships across the Health and Social Care Partnership, partner agencies and 3rd sector organisations
