Job Responsibility:
Conducting holistic and person-centred social work assessments of older adults' needs, considering their physical, emotional, social, and environmental circumstances
Developing, implementing, and reviewing tailored care plans in collaboration with service users and their support networks
Safeguarding vulnerable adults by confidently applying Adult Protection procedures and legislation
Managing a complex caseload and prioritising effectively to meet the diverse needs of service users
Working autonomously and as part of a multidisciplinary team, fostering effective communication and collaboration with health professionals, housing providers, and other relevant agencies
Undertaking duty work, responding to urgent referrals and providing timely and appropriate interventions
Maintaining accurate and up-to-date records in accordance with relevant policies and procedures
Contributing to the ongoing development of the Older People's Team and participating in team meetings and professional development opportunities
Requirements:
A recognised Social Work qualification (e.g., BA/MA in Social Work, DipSW)
Registration with the Scottish Social Services Council (SSSC) as a Social Worker
A full driving licence and access to a vehicle
