Job Description:

We are seeking a dedicated and motivated Social Work Officer to join our Mental Health Team. This is an exciting opportunity to make a real difference in the lives of adults living with mental health conditions. The successful candidate will play a key role in delivering high-quality social care interventions in line with statutory duties and local authority functions.

Job Responsibility:

  • Carry out Care Act Assessments and Reviews for adults with mental health needs
  • Manage a caseload effectively, ensuring timely and appropriate interventions
  • Work as part of a supportive team, attending team meetings and contributing to collaborative decision-making
  • Represent social care within a multi-professional setting, working closely with clinical teams and attending relevant professional meetings
  • Undertake welfare checks, home visits, and other duties aligned with local authority responsibilities
  • Work flexibly to meet the needs of adults requiring social care support
  • Complete written assessments to a high standard, demonstrating strong written and verbal communication skills
  • Ensure compliance with statutory timeframes and guidelines
  • Manage competing demands effectively and maintain accurate records

Requirements:

  • Strong understanding of the Care Act 2014 and its application in mental health settings
  • Ability to work collaboratively with professionals across health and social care
  • Excellent organisational skills and the ability to manage a varied caseload
  • Skilled in critical thinking, problem-solving, and maintaining professional boundaries
  • Commitment to promoting independence, wellbeing, and safeguarding adults
