Job Description

Our groundbreaking hospice and palliative care programs offer a significant difference when dealing with a life-limiting condition. We offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.

Job Responsibility

  • Responsible for managing the Hospice and Palliative Care Social Workers by providing leadership and mentoring guidance to the staff on a day-to-day basis to ensure the provision of Hospice and Palliative Care services are consistent with the philosophy, goals, and policies of the program and MJHS
  • Coordinates the provision of Hospice and Palliative Care psychosocial services
  • Ensures adequate staffing and appropriate patient assignments
  • Maintains the integration of psychosocial services as part of the Hospice and Palliative Care program's interdisciplinary team approach
  • Functions as a member of the Hospice and Palliative Care management team and attends management meetings as directed

Requirements

  • Master's Degree in Social Work from a Graduate School of Social Work accredited by the Counsel of Social Work Education
  • Minimum five years post Master's degree
  • minimum 2 years end-of-life care experience
  • minimum 2 years of supervisory/program development experience
  • New York State LCSW licensure within one year of employment
  • SIFI Certification within one year of employment

Nice to have

  • R # preferred
  • valid NYS Drivers license preferred
