Job Description

Baptist Health is looking for a Social Services Team Lead at our Baptist Beaches campus. This is a Full Time opportunity. The Social Services Team Lead will assess psychosocial needs and provide services that enhance patient functioning, emotional well-being and continued access to services and resources in the community. Provide discharge planning, arrange home health and DME, assist with placement/transfers, provide referrals to community services.

  • Social work resource for team members and care coordination program
  • Provide guidance through challenging situations
  • Promote problem-solving/critical thinking skills among team members to be utilized upon new challenges/barriers
  • Assist with interviewing and hiring staff members who will enhance the care coordination program’s ability to meet its goals and objectives
  • Coordinating orientation and training for newly hired staff
