Job Responsibility:

  • Conduct social and emotional assessments of residents to identify their needs and develop personalized care plans
  • Develop and implement individualized care plans in collaboration with interdisciplinary team members
  • Provide emotional support and counseling to residents and their families
  • Facilitate connections with local community resources and services to meet residents' needs
  • Maintain accurate and comprehensive documentation of resident assessments, care plans, and service delivery
  • Participate in quality improvement initiatives to enhance resident care and services

Requirements:

  • Prior experience as a social worker, preferably in a long-term care setting
  • Bachelor's degree in Social Work, Psychology, or related field
  • Valid driver's license and reliable transportation
  • Strong communication and interpersonal skills
  • Ability to work effectively with diverse populations and maintain confidentiality
  • Strong organizational and time management skills

  • Work as part of the national team, provide support to asylum seekers within all accommodation types across the UK
  • Support Asylum Seekers to complete applications when required, ensuring forms are completed accurately, with the correct level of detail and within required KPIs and timescales
