Social Care Team Leader for Supported Living Services | Adults & Wellbeing Directorate. Community-Based Role. Are you an experienced care professional ready to take the next step into leadership? We’re looking for a Social Care Team Leader to join a high-performing, person-centred Supported Living Service, supporting adults with learning disabilities to live independently, safely, and with dignity in their own homes. About the Role As a Team Leader, you’ll play a key role in the day-to-day running of the service, supporting the Registered Manager while leading and developing a team of Support Workers. This is a hands-on leadership role where you’ll: Drive high-quality, person-centred care; Support individuals to live fulfilling, independent lives; Ensure services are safe, compliant, and outcome-focused; Lead by example and inspire your team to deliver their best every day
Job Responsibility:
Supervise and support a team of Support Workers to deliver consistent, high-quality care
Coordinate staffing and resources to ensure smooth daily service delivery
Carry out supervisions, performance reviews, and development planning
Ensure care plans and risk assessments are person-centred, accurate, and up to date
Promote and maintain safe working practices and safeguarding standards
Oversee medication administration to ensure safety and compliance
Build strong relationships with service users, families, and professionals
Support service quality through reviews, audits, and continuous improvement
Participate in the on-call rota, including weekends and bank holidays
Requirements:
A Level 3 qualification in Care (or equivalent)
Recent experience in a supervisory role within a care setting
Strong knowledge of: Safeguarding and risk management
Person-centred care planning
Health & safety and regulatory requirements
Medication administration
Experience leading teams working on a rota basis
The ability to motivate, develop and support staff
Excellent communication and problem-solving skills
Confidence using digital care systems and recording tools
A genuine commitment to equality, inclusion, and independence
Enhanced DBS (including Adults Barred List)
Full UK driving licence / car user
Flexibility to take part in an on-call rota
