As a Social Care Assistant working within the Home Independence Team, you will be working out in the community to support residents that have been recently discharged from hospital or those who are living with a disability and need help to live independently and establish a routine to help them manage their lives day to day. You will be supporting individuals to regain confidence and reach their goals of living independently again.
Job Responsibility:
Provide people with information and advice in relation to care and support and assistance in accessing care and support through various means
Work as part a CQC Registered service in the community, maintaining a high degree of autonomy and utilising own initiative in delivering compliant care and support independently
Ensure an agreed plan meets individual’s care and support outcomes needs and to empower the individual to access early intervention and preventative services
Inform senior colleagues and adhere to safeguarding procedure of any concerns which arise in relation to the safety of vulnerable adults
Work in partnership with individuals, carers, service providers, voluntary sector and other professionals
Provide personal care for individuals, this may include but is not limited to dressing, toileting, washing and general housekeeping
Requirements:
Full UK driving licence
Access to a vehicle
Great people person

Able to deliver practical care and support to an agreed plan
Ability to use own initiative to problem solve
Ability to utilise remote management oversight where required
May need to undertake a specialist role requiring specific training (e.g., Autism services, administering medication)
Must be able to declare all unspent convictions and conditional cautions
Must be able to declare all spent convictions and adult cautions that are not protected
Must currently reside in the UK with established proof of right to work documentation
