As a Social Care Assistant working within the Home Independence Team, you will be working out in the community to support residents that have been recently discharged from hospital or those who are living with a disability and need help to live independently and establish a routine to help them manage their lives day to day. You will be supporting individuals to regain confidence and reach their goals of living independently again. The Home Independence service supports individuals to regain their independence following illness, disability, or recent stay in hospital. We work with clients in their homes, identifying whether they now have difficulties with self-care or performing daily activities. We provide a goal-based assessment for each client and alongside them to support the achievement of their goals, using technology-enabled care devices and occupational therapy aids to enable them to achieve independence. The team works closely with healthcare professionals, carers, clients and their families to develop suitable packages of support, designed to enhance client independence and meet their needs.
Job Responsibility:

Assist in accessing care and support through various means
Work as part a CQC Registered service in the community
Maintain a high degree of autonomy
Utilise own initiative in delivering compliant care and support independently
Ensure an agreed plan meets individual’s care and support outcomes needs
Empower the individual to access early intervention and preventative services
Inform senior colleagues and adhere to safeguarding procedure of any concerns which arise in relation to the safety of vulnerable adults
Work in partnership with individuals, carers, service providers, voluntary sector and other professionals
Provide personal care for individuals (e.g., dressing, toileting, washing, general housekeeping)
Requirements:
Full UK driving licence
Access to a vehicle

Able to deliver practical care and support to an agreed plan
Use own initiative to problem solve
Ability to work with remote management oversight
May need to undertake specialist role requiring specific training (e.g., Autism services, administering medication)
Full training will be provided
All unspent convictions and conditional cautions must be declared

Candidates must currently reside in the UK with established proof of right to work documentation
