Working within the Home Independence Team to support residents recently discharged from hospital or living with a disability to live independently and establish a routine. Supporting individuals to regain confidence and reach their goals of living independently again. Providing high quality direct care and support to residents, focusing on personal wellbeing outcomes and individual wishes, to achieve strength-based support.
Job Responsibility:
Provide information and advice in relation to care and support
Assist in accessing care and support
Work as part of a CQC Registered service in the community
Maintain a high degree of autonomy
Utilise own initiative in delivering compliant care and support independently
Ensure an agreed plan meets individual’s care and support outcomes needs
Empower the individual to access early intervention and preventative services
Inform senior colleagues and adhere to safeguarding procedure of any safety concerns
Work in partnership with individuals, carers, service providers, voluntary sector and other professionals
Provide personal care for individuals (e.g., dressing, toileting, washing, general housekeeping)
Requirements:
Full UK driving licence
Access to a vehicle
Great people person
Caring and empathetic
Able to deliver practical care and support to an agreed plan
Use own initiative to problem solve
May need to undertake a specialist role requiring specific training (e.g., Autism services, administering medication)
