Job Description:

We’re looking for Social Care Assessors to join our team in Manchester, helping vulnerable adults and families access the right support to live healthier, more independent, and fulfilling lives.

Job Responsibility:

  • Undertake holistic, strengths-based assessments in the community, hospitals, and intermediate care facilities
  • Arrange support such as reablement, care packages, respite, equipment, minor adaptations, and assistive technology
  • Develop and implement personalised support plans to meet social care needs
  • Identify safeguarding and mental capacity issues, referring appropriately
  • Collaborate with partner agencies to promote independence and positive outcomes

Requirements:

  • Strong communication, analytical, and planning skills
  • Ability to work collaboratively across multiple teams
  • Commitment to continuous learning and self-development
  • NVQ Level 4 in Health and Social Care (or willing to achieve within 6 months)
  • Enhanced DBS clearance (or willingness to obtain)
