Job Description:

Join our team-oriented group of compassionate clinicians in Metro Charlotte, where supportive leadership, strong work-life balance, and the opportunity to deliver exceptional patient care come together. Manage complex patients with ongoing follow-up throughout stay. Collaborate with SNF team to facilitate care coordination. Implement discharge planning to improve patient outcomes.

Requirements:

  • Physician Assistant or Nurse Practitioner
  • Passion for working in a SNF/Post-Acute Care Environment
  • Experience in a Skilled Nursing Facility is preferred but not required
