Bachelor's degree from an accredited school of pharmacy with Board Certification in Geriatric Pharmacy (BCGP) or a Pharm D
Current State Board of Pharmacy license required for state of practice
Current knowledge and the ability to advise uses of medications, multiple prescriptions, side effects and make potential problems known to physicians
Must have BGCP or if qualifications are met, must obtain BCGP within 30 months of date of hire

Job Responsibility

Monitors the medication usage for participants to ensure it is safe, effective, cost efficient and contributes to optimal participant outcomes
Acts as a drug information resource for the medical and nursing teams, participants, and their families
Evaluates medication regimen of participants
Evaluates and reports drug interactions
Assists primary care team in maximizing drug therapy outcomes
Reviews participant's medication list and makes recommendations
Alerts primary care to dosing irregularities
Monitors response to drug therapy
Optimizes drug therapy against disease states
Reviews dementia patients on psychotropic medications
