We have an exciting opportunity for a Service Lead to join our team at our supported living service in Brotton. Reporting to the Deputy Manager, the purpose of a Service Lead is to lead a cluster of support services that provides agreed level of support to the people we support, in order to achieve their individual goals, and to lead a full and productive lives as valued member of their community. You will be supporting our Service Users to achieve their individual goals and lead full and productive lives as valued members of their community, while ensuring services provided adhere to Real Life Options' values. Job Responsibility:
Lead a cluster of support services that provides agreed level of support to the people we support
Manage 2 supported living services and a small outreach service in the Manager's absence
Complete rotas and carry out staff supervision
Attend reviews with People we Support and update support plans, risk assessments
Complete medication audits, finance audits
Ensure that all PWS are safeguarded with support from the Manager
Act as a role model for staff teams
Take responsibility for the day to day running of activities
Attend, participate and lead within team meetings and service user reviews
Ensure medication procedures are in place
Ensure activity timetables are upheld and updated
Ensure service users are supported appropriately
Undertake specified managerial responsibilities
Support on shift where needed
Play a key role in the assessment of service users and the development of a Person Centred Plan
Help facilitate the participation of all support workers in implementation of the agreed plan
Requirements:
Level 3 in health and social care
Demonstrable track record of working with people with learning disabilities in an enabling environment
Good understanding and commitment to the principles of Person Centred Care
Understanding of the needs of people of the relevant service use group, e.g. Learning Disabilities, Mental Health, Complex Care, Autism etc.
