Service Access Coordinator

• Be the first point of contact for families and professionals making referrals
• Support young people and their families through the onboarding process and help them with a positive introduction in our services
• Prepare key onboarding documentation including initial support plans, risk assessments and PEEPs
• Coordinate bookings for After-School Clubs, Weekend and Holiday Clubs
• Maintain accurate attendance registers and monitor usage
• Identify wider family needs and work collaboratively with our family service
• Liaise with schools, SENCOs, social workers and health professionals as required

• Experience working within children’s services, SEND, education, social care, out-of-school or family support or similar
• Understands SEND, learning disability or willing to learn
• Experience gathering structured information for assessments, support plans or risk assessments
• Can manage multiple deadlines and maintain high accuracy in data entry
• Confident using digital systems
• Works calmly under pressure and remains solution-focused
• Builds positive relationships with families and external partners

A Level 3 qualification in childcare, youth work, education, administration or a related field
