We are seeking a highly skilled and motivated Senior Staff Nurse (SSN) to join our Virtual Ward team. This is a dynamic and rewarding role that offers the opportunity to work at the interface of community and secondary care, managing acutely unwell patients with complex needs. The SSN will be instrumental in delivering safe, effective, and person-centred care to patients who would otherwise require hospital admission.
Job Responsibility:
Triaging referrals to the Virtual Ward Hub
Conducting comprehensive assessments, physical examinations, and diagnostic investigations
Managing acute and long-term conditions, including frailty, respiratory illness, and palliative care needs
Liaising with GPs, pharmacists, district nurses, social workers, and specialty teams across the Integrated Care System (ICS)
Supporting seamless care transitions between acute and community settings
Contributing to the development of virtual ward pathways and service redesign
Participating in departmental meetings and representing the service as required
Supporting the development of colleagues and students through teaching, supervision, and mentorship
Promoting a culture of learning and reflective practice
Planning, co-ordinating and delivering care as appropriate per the daily MDT
To provide high quality holistic evidence-based care
To facilitate and support timely assessment, treatment and effective discharge
To holistically assess the needs of patients and carers, plan, implement and evaluate nursing care interventions
Undertake virtual monitoring and escalate concerns to the daily MDT
Take responsibility for leading the shift on a regular basis
To deliver high quality care to patients / service user on the community virtual ward
To assess, plan and implement treatment programmes
To provide education and advice service to patients / service users promoting independence and self-management techniques
To lead and co-ordinate timely discharge from the virtual ward and prevent inappropriate hospital admissions
To act as Role Model to all team members
Maintain accurate and contemporaneous documentation
Be personally accountable for maintaining skills, knowledge and competence in the use of SystmOne
Champion the roles and values of the virtual ward
Self-evaluate practice to ensure evidence-based care
Contribute to the wider development of practice by participating in research, audit, local and national presentations, networks, and publications
Educate and empower patients, carers, and care home staff
Develop robust care plans with patients and carers
Participate in the future training of health students and staff and other professional groups
Effectively communicate with the daily MDT, virtual ward colleagues, wider Integrated Teams, patients, and carers
Facilitate effective communication between Primary, Secondary, Community, Social Care and Voluntary settings
Maintain high standards of documentation
Ensure the implementation of the Virtual Ward concept
Actively engage in clinical supervision and mentorship
Maintain a personal development plan and professional portfolio
Maintains professional development, encompassing mandatory requirements
Ensure revalidation is maintained
To be responsible for the promotion of best practice that ensures the health, safety and security of self, patients, and colleagues
Practice in accordance with the procedures and guidelines for the protection of vulnerable adults and child protection
Ensure that confidentiality is protected at all times
Requirements:
Level 1 Registered General Nurse/AHP with extensive post registration experience
Substantial post registration experience
Evidence of Teaching and assessing in clinical knowledge
Evidence of ability to plan and co-ordinate programmes of care working autonomously and collaboratively
Evidence of effective team working
Experience of working between primary and secondary care settings
Experience of managing long term conditions
Evidence of managing acute conditions
Understanding of local and national priorities in relation to acute/long term patient management
Evidence of ability to communicate both verbally and in writing at all levels in multi-disciplinary forums and with users and carers
Experience of teaching and dissemination of knowledge to enhance the skills of others
Evidence of the ability to perform effectively in a changing, stressful situation
Current UK professional registration
Nice to have:
Relevant Honours degree
Completion of the non-medical prescribing course (V300)
