The Senior QSW (Health Liaison Worker) will be responsible for working directly with adult clients, health professionals, and care teams to coordinate care, facilitate communication, and ensure the delivery of appropriate services. This role requires a proactive individual with strong assessment skills, excellent communication, and experience working within health and social care environments.
Job Responsibility
Act as a key point of contact between health services, social care teams, and adult clients to ensure seamless care coordination
Conduct thorough assessments to identify clients’ needs, risks, and eligibility for support
Develop personalized care plans tailored to individual client requirements
Facilitate effective communication among multi-disciplinary teams to promote client well-being
Support the delivery of services that enable independence and improve overall health outcomes
Maintain accurate records and ensure compliance with safeguarding, confidentiality, and data protection policies
Lead case reviews and contribute to team discussions on complex cases
Provide advice, guidance, and support to less experienced colleagues within the team
Requirements
Proven experience working as a Qualified Social Worker (QSW) within adult health and social care
Strong knowledge of adult safeguarding, mental capacity, and care planning
Excellent interpersonal and communication skills
Ability to work independently and as part of a multidisciplinary team
Relevant social work qualification and registration with Social Work England
Experience in health liaison or care coordination is desirable
