B.S. Degree or Pharm.D. in Pharmacy from an accredited educational institution
Three years of experience as a practicing pharmacist
Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the ambulatory care patient in collaboration with physicians, nurses, and other healthcare professionals
Current Minnesota pharmacist license
