Deliver professional Occupational Therapy services in line with local authority policies, national legislation, and the Royal College of Occupational Therapy (RCOT) 4 Pillars
Undertake complex, strength-based and person-centred assessments in collaboration with service users, carers, and multidisciplinary teams
Recommend and provide equipment and adaptations to maximise independence and improve quality of life for service users and carers
Develop and monitor SMART goals to support outcome-focused interventions
Facilitate hospital discharge planning, step-down and reablement pathways, community-based interventions
Work collaboratively across health and social care services to prevent hospital admissions, enable safe and timely discharges, support individuals transitioning back to independent living
Assess and support individuals with long-term conditions, recommending specialist equipment and adaptations where required
Requirements
Qualified Occupational Therapist with HCPC registration
Experience managing complex caseloads within social care/community settings
Strong knowledge of manual handling, equipment provision, and housing adaptations
