This role is part of Peterborough City Council's Reablement Team's new 1 year Learning Disability Project, aiming to maximise individuals' wellbeing and independent living skills. The work is based in the community, supporting people with a Learning Disability (LD) who may have complex communication difficulties, sensory needs, behaviours of concern, health and social care needs and physical disabilities.
Job Responsibility:
Maximise individual's wellbeing and independent living skills as part of a 1 year Learning Disability Project
Support people through their reablement time
Work with colleagues to support individuals to achieve their goals
Have responsibility & line management for Care Support Workers
Work as part of a multidisciplinary team supported by psychology, nursing, speech & language therapy and social workers
Support the Reablement Team Manager and a Community Long Term Team Manager in service development and management of the day to day running of the LD Reablement Service
Deliver holistic assessments and improve the quality of life for the people of Peterborough
Promote the principles of personalisation, encouraging independence, choice and opportunities
Requirements:
Bachelor's or Master's degree in Occupational Therapy
Registered with the HCPC
Experience working as a qualified Occupational Therapist
Strong communication and interpersonal skills
Ability to work effectively both independently and as part of a team
Commitment to continuous professional development
Own transport to travel between community locations
Experience and/or knowledge of the needs of vulnerable adults living with a learning disability or neurodivergence
Working knowledge and experience of the Care Act & Eligibility Criteria and Mental Capacity Act
Excellent interpersonal and organisational skills
Problem solving and positive attitude
Independent, well-reasoned judgement and decision making skills
Highly specialist skills to assess the impact of an individual's learning disabilities on their occupational performance
Ability to implement, monitor and evaluate highly specialist treatment programmes
Ability to work collaboratively with other health and social care professionals
