We are looking for a dedicated and experienced Senior Clinical Quality Registered Nurse (RN) to join our team in Eden Prairie, Minnesota. In this long-term contract position, you will play a critical role in delivering telephonic and digital patient care strategies aimed at improving health outcomes and closing gaps in care for Medicare patients. You will collaborate with an interdisciplinary team to provide patient-centered clinical consultations and support the operational goals of the organization.
Job Responsibility:
Conduct outbound and inbound calls with patients, caregivers, and healthcare providers while adhering to established protocols and scripts
Perform comprehensive patient assessments, including Care for Older Adults and functional assessments
Educate patients about health conditions, the importance of routine screenings, and assist with scheduling appointments for services such as A1c tests, blood pressure monitoring, breast cancer screenings, and diabetic eye exams
Coordinate with healthcare providers and offices to gather documentation and evidence of completed lab work and medical screenings
Help patients navigate community resources and programs to address non-medical needs
Maintain detailed and accurate documentation of all interactions and actions in the organization's core systems
Meet or exceed productivity and quality metrics while supporting business hours, Monday through Friday, during Central Time
Collaborate with the Director of Clinical Programs and interdisciplinary team members to optimize patient care strategies
Perform additional duties as assigned to support the operations and objectives of the organization
Requirements:
Active and unrestricted Registered Nurse (RN) license with eNLC Compact licensure in the state of residence
Minimum of two years of experience as a Registered Nurse with attention to detail
Extensive knowledge and experience with Medicare Stars measures
Proven ability to manage multiple complex projects simultaneously
Proficiency in Microsoft Office applications, including intermediate or advanced skills in Excel
Strong communication and customer service skills
Ability to work independently while meeting productivity and quality standards
Nice to have:
Familiarity with telephonic and digital healthcare engagement strategies is highly desirable
