Job Description:

Nurses in the Center for Nursing Entrepreneurship (CNE) provide direct care services to individuals and groups in a variety of settings. This position involves supporting a variety of different roles, based on departmental needs and seasonality of services, including biometric screenings, lifestyle and condition management coaching, and seasonal flu vaccinations.

Job Responsibility:

  • Provide direct care services to individuals and groups in a variety of settings
  • Support roles including biometric screenings, lifestyle and condition management coaching, and seasonal flu vaccinations
  • Perform full panel biometric screenings at employer sites
  • Provide real time follow up counseling and education
  • Refer/recommend clients to appropriate programs
  • Provide holistic care within a robust coaching model
  • Manage a client case load and provide 1:1 coaching and counseling
  • Provide comprehensive influenza vaccination services
  • Conduct POCT biometric screenings
  • Interpret results to understand the client’s risk profile
  • Make referrals as appropriate
  • Use technology effectively to meet client needs
  • Communicate with other service providers
  • Identify critical clinical situations and intervene
  • Deliver care in an autonomous and competency supportive environment
  • Integrate information in real time to develop individualized recommendations
  • Assess the impact of the clients’ current state of health
  • Use critical thinking to assess, intervene and evaluate needs
  • Practice cultural agility
  • Apply motivational theory to encourage behavior change
  • Utilize expert coaching techniques
  • Engage and encourage participant to utilize resources
  • Knowledgeable regarding immunization standard of care practices
  • Review medical questionnaire/Conduct medical evaluation
  • Provide education and instruction on influenza vaccine
  • Conduct and coordinate intake assessments, ongoing appointments and program completions
  • Develop an individualized wellness plan
  • Monitor the progress of the client towards goal achievement
  • Utilize evidence based practice and best practice models
  • Evaluate client outcomes at end of program
  • Develop presentations and group programs as requested
  • Delivers group programs and presentations
  • Uses sound adult learning educational principles
  • Uses various modalities to deliver content
  • Evaluates effectiveness of group programs/ presentations
  • Stay current with best practices
  • Participates in quality improvement as requested

Requirements:

  • Baccalaureate prepared Registered Nurse
  • Current CPR certification
  • 2 years related experience
  • Licensed Registered Nurse in NYS
  • Wellcoaches® certification (or equivalent wellness coaching certification) required within one year of hire for PT and FT staff
  • Ability to apply evidenced based and theory based assessments, interventions and evaluations to clinical situations
  • Ability to work independently, exercise independent nursing judgement and use critical thinking abilities
  • Regular daily travel is an essential requirement
  • Candidates must have a reliable means of transportation
  • Basic computer skills are essential

Nice to have:

  • Expertise in adult nursing with 3 to 5 years’ experience preferred
  • Specialty certification desirable
