This position covers the East Valley area. As a Home Health Weekend RN, you will be a clinical leader applying expertise in a patient's home, crafting care plans, collaborating with a team, empowering patients, mentoring staff, and using technology.

Work seamlessly with a multidisciplinary team – physicians, therapists, and support staff – to ensure holistic care

Serve as a vital resource for patients and their families, providing crucial teaching and emotional support
Mentor and guide
Oversee your caseload, leading and nurturing LPNs, LVNs, and Home Health Aides to foster a strong, cohesive care team
Innovate with technology
Utilize leading-edge tools and best practices to optimize care delivery and maintain precise documentation
Requirements:
Graduate of an accredited nursing program with an active RN license in the state served
Minimum of 1 year of Home Health nursing experience
Exceptional critical thinking, proven clinical expertise, strong case management abilities, and an unwavering commitment to patient-centered care
Current CPR/BLS certification
Valid driver’s license and reliable transportation
