As our RN Transition Fellow, you will provide professional nursing care to patients of all ages, adhering to individual needs, physician orders, Hospital policy, and nursing standards. You will also assume responsibility for directing and coordinating care team members, focusing on individualized quality patient care consistent with organizational values. Every day, you will collaborate with the healthcare team to implement comprehensive patient care plans and participate in direct patient care activities, including assessment, medication administration, documentation, and patient education across diverse patient populations.
Job Responsibility:
Provide professional nursing care to patients of all ages, adhering to individual needs, physician orders, Hospital policy, and nursing standards
Assume responsibility for directing and coordinating care team members, focusing on individualized quality patient care
Collaborate with the healthcare team to implement comprehensive patient care plans
Participate in direct patient care activities, including assessment, medication administration, documentation, and patient education across diverse patient populations
Adhere to established Hospital/departmental policies and procedures
Apply clinical knowledge and expertise for coordinating patient care by regularly planning and evaluating the care of patients
Establish and maintain professional and collaborative relationships with other staff members, physicians, patients, patient family members, and the public
Demonstrate effective nursing judgement and a working knowledge of the nursing process
Provide guidance to interdisciplinary team members by supervising nursing interventions and assigning related tasks
Provide leadership by establishing priorities and coordinating nursing care
Document and communicate all observed symptoms, reactions, treatments and changes in the patient condition to the appropriate member of the interdisciplinary health care team
Provide for teaching based on identified need
Coordinate plan of care with other members of the interdisciplinary health care team to achieve desired goals and outcomes
Communicate the plan of care to the patient and advocate for the active involvement of the patient
Conform to legal requirements of nursing practice and Hospital policies and procedures
Demonstrate a professional, cooperative manner when dealing with all members of the interdisciplinary health care team
Demonstrate understanding of organizational structure and utilize appropriate channels of communication
Administer medications to assigned patients as directed and assess and document patient responses
initiate appropriate interventions per Hospital/Unit policy
Demonstrate an understanding of the approaches to and methods of Continuous Performance Improvement
Assign duties and oversee unlicensed assistive personnel based on legal scope of practice, regulations, and hospital policy
Collaborate with the supervisory staff to identify own educational and professional growth needs
Function under the supervision of a Registered Nurse Preceptor until successful completion of a RN transition fellow orientation program is achieved
Actively participate in a structured preceptorship during the orientation/residency program
Seek out learning opportunities in conjunction with their assigned preceptor
Attend and successfully complete all core and specialty classes as required
Complete evaluation processes, residency metrics, and competency validations in a timely manner
Demonstrate an understanding of and ability to follow the chain of command
Actively participate in required looping experiences, mentorship program and debriefing sessions in accordance with defined timelines
Requirements:
Graduate of Accredited RN school of Nursing
Minimum 2 years of RN experience in an acute care setting, upon hire
