The Home Health Admission Nurse is a licensed professional responsible for performing comprehensive patient admissions and completing OASIS assessments for individuals requiring skilled nursing services in the home setting. This role also includes the delivery of direct skilled nursing care in accordance with physician orders.Working collaboratively with an interdisciplinary home health team, the Admission Nurse ensures high‑quality, patient‑centered care under the guidance and supervision of the Clinical Supervisor. Key responsibilities include evaluating patient needs, establishing care plans, coordinating services, and supporting continuity of care throughout the home health episode.This position follows a four‑day workweek, consisting of 10‑hour shifts, and includes a requirement to work one weekend per month and two holidays per year.
Job Responsibility:
Assess and admit eligible patients for home health care services
Conduct comprehensive OASIS assessments in accordance with regulatory and organizational standards
Evaluate patient needs and determine appropriate levels of home health services
Develop individualized plans of care based on patient assessments, physician orders, and clinical standards
Implement, monitor, and revise care plans to ensure high‑quality, safe, and effective patient outcomes
Provide skilled nursing services and comprehensive clinical assessments in the home setting
Perform and manage physician‑ordered treatments including: PleurX drain care
Finger-stick coagulation checks
Wound care and wound VAC management
IV therapy
Phlebotomy
Tracheostomy care
PEG tube care
Ostomy care
Telehealth monitoring and support
Educate clients and caregivers on disease management, medication administration, treatments, and safety
Provide supervision and clinical guidance to patients and family members to promote independence and self‑care
Offer counseling to support patient understanding and participation in the care plan
Work collaboratively with members of the multidisciplinary home health team to ensure coordinated, patient‑centered care
Communicate effectively with physicians, therapists, social workers, and supervisors regarding patient status and care needs
Complete accurate, timely OASIS documentation in compliance with regulatory and agency standards
Maintain thorough and clear clinical records to support quality care and continuity of services
Utilize strong critical thinking and clinical judgment to identify patient needs and respond effectively
Address patient and caregiver concerns promptly, employing sound problem‑solving strategies
Participate in required weekend and holiday rotations
Perform other duties as assigned to support optimal patient care and agency operations
Requirements:
Minimum of one (1) years recent experience as a professional nurse in Home Health
Valid DL With No More Than 3 Moving Violations
RN - Registered Nurse
BLS - Basic Life Saving certification (American Heart Association)
Graduate of an accredited school of nursing
