Ambercare Hospice is seeking a compassionate and dedicated Registered Nurse - On Call to join our newly restructured team in Silver City, NM. As part of the Addus HomeCare family, we serve thousands of patients across the region—combining heart, professionalism, and the highest standards of care. 7 days on and 7 days off for a work schedule.

Job Responsibility

  • Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
  • Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
  • Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
  • Provide regular visits to re-assess the patient's needs and update the plan of care
  • Perform clinical pronouncement of death and provide support to the family
  • Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
  • Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines

Requirements

  • Graduate from an accredited registered nursing program
  • Licensed as a registered nurse in the state of practice
  • Must possess current CPR, First Aid Certification
  • Must be knowledgeable of quality assessment and assurance procedures
  • Valid driver’s license and proof of insurance is required

  • Available via cell phone to respond to Triage visit requests, and communicates effectively with the on call team, including Hospice physician to address urgent patient needs, and establish a safe plan of care
  • Implements plan of care, educating patient/primary caregiver on changes to plan of care, including medication management, patient safety, and symptom management
  • Attends patient deaths as directed to provide emotional support and guidance to family
  • Reports off to triage and primary Hospice team concerning patient related visit, changes to plan of care, and follow up needs
  • Follows NYS Standards for Professional Registered Nurse

  • Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.)
  • Provides appropriate support at time of death and perform bereavement assessment
