The person in this role provides RN - Nurse Case Management to pregnant people and their exposed infants, children, youth (ages 17-25), and women living with HIV. Serves as a subject matter expert in the care of people living with HIV and as an expert consultant to health care providers within Southeastern Wisconsin, and surrounding areas. Monday-Friday (40 hrs per week) No nights or weekends!
Job Responsibility:
Provides RN - Nurse Case Management to pregnant people and their exposed infants, children, youth (ages 17-25), and women living with HIV
Serves as a subject matter expert in the care of people living with HIV and as an expert consultant to health care providers within Southeastern Wisconsin, and surrounding areas
Has advanced knowledge and expertise in the understanding and implementation of the care needs of patients with HIV infection
Coordinates the care of patients across the continuum of healthcare settings including hospital, clinic, and the community
Collaborates with multiple providers working closely with the HIV specialty physician, HIV social worker, HIV multi-disciplinary care team, and community providers to assess needs, plan, and implement care to patients
Requirements:
Bachelor's degree in Nursing required
1 year nursing experience required
RN license for the State of Wisconsin
Current CPR certification - Basic Life Support (BLS) for Healthcare Providers through American Heart or American Red Cross is required
Driver's License - State of Wisconsin required
Good driving record with adequate insurance coverage required
Compassion and commitment to caring for people living with poverty, mental illness, drug use, LGBTQ issues, and many complex social needs
Ability to work flexible hours to meet patient care needs
Analytical skills necessary to resolve complex patient problems, develop standards of care and procedures, and evaluation of the quality of nursing care
Interpersonal communication skills sufficient to interact effectively with clients and their families, other CHW departments, and interdisciplinary health team members within the community
Ability to communicate effectively via telephone, voice mail, e-mail
Ability to organize work effectively and multitask
Detail oriented, demonstrated ability to track detailed data required for patient's longitudinal care
Nice to have:
Experience in managing patients with chronic and complex health care needs preferred
Experience in the care of patients with HIV preferred
